And the doctor dreams of real reforms in the field of medicine and, together with like -minded people, is working to create palliative care for seriously ill patients. My task, as a pediatrician, is not just to eliminate pain in the ear or tummy, but to understand where it came from. Then it is important to convey to their parents the reason and help them solve the problem either with the help of pediatrics, or attracting a child psychotherapist, other specialists.
They say that all diseases are from the nerves. Diseases in children, as in adults, sometimes appear not from colds or disorders of the liver, but from fears and problems that are formed at an early age. And the reason for these fears is a relationship in the family, scandals, a lack of love, a lack of time for parents for a child ... Perhaps this will seem strange to someone, but where I work, we try to follow international pediatric standards that, alas, have not taken root in our country.
In the USA, from where I recently returned, the children in Palliative live for decades with adequate assistance and normal medical support and die their death in human conditions. This is my 7th or 8th trip, during which I have the opportunity to get acquainted with the experience of various medical centers. I would like to tell how my travels began. Somewhere in the year, the American Carmel, who adopted two children in Kharkov, arrived in one of the local houses of the child with gifts, but she was not allowed.
When she called Kyiv, she was advised to go to us, so we made friends. Since our child’s house is specialized, the purpose of trips to the United States was to see the experience of working with children who have multiple developmental disabilities. This time it was palliative help, because we saw that the trajectory of the disease and life in children with multiple characteristics and deficiency has a very negative prognosis.
Since the beginning of the year, three pupils from our house have died in the hospital, who suffered from incurable diseases. Abroad, everything is done to increase the sensations of comfort and pleasure from life with severe patients. Palliative experts there not only help to cope with pain or relieve symptoms of fear, nausea, pressure sores, butter, but also support in solving psychological, spiritual and social needs of both the patient and his family.
Imagine, patients with a vegetative status who, it would seem, lie and do not feel anything about, they are being taken to the city in the evening so that they can look at the illumination, arrange them holidays and performances, spa treatments, give gifts. There are specialists in the development of alternative speech. By pressing the button on the gadget, the patient can, as it were, say or write a certain phrase.
Everything is done so that a person even in such a serious condition feels as comfortable as possible. Telemedicine centers are no longer fantasy, but reality. In America, online medicin is very in demand, because carrying seriously ill children to bring a child to an appointment to a doctor is much more expensive than installing such a system-it pays off very quickly.
Telemedicine is far reduced by the process of medical care. And most importantly: you can instantly solve the question with a specialist who exciting you as soon as the problem has appeared. The center of telemedicine at the school for the disabled was struck by such a center of telemedicine. The doctor not only sees the patient's face, but can also familiarize themselves with the parameters that are removed on the other side of the screen.
A phonondoscope is connected to the system and the doctor hears how the heart beats, the lungs breathe. Then the otoscope is turned on, with which the doctor can see in what condition the drum membranes, the pharynx are located. The whole study is conducted online. Thanks to new approaches to treatment, the results are completely different. When the first time I saw a dog in the American hospital, it did not surprise me at all.
With their attitude to the process and the depth of understanding of problems - it looked quite natural. I don’t know why dogs are not allowed into our hospitals. Most of the problems in Ukraine, it seems to me, because of our mentality. After all, it has long been clear that the well -groomed dog will not bring any danger to the medical institution, but on the contrary, there will be only additional health benefits.
This time during a trip to the United States, wherever I come, I met dogs everywhere. A huge number of studies indicate a positive effect of petot therapy method of treating patients with pets-dogs, horses, rabbits, cats, birds-ed. The meaning is the same: to create a child wherever he is - in a rehabilitation department or resuscitation - a maximum of positive emotions and impressions that will contribute to recovery.
The Americans do not believe that their palliative help is at the highest level. They are actually building a system: in each medical institution they organize the so -called integrative teams of palliative care, create ligaments between different services and prescribe the “route” of the patient. The approach, as always, is very thorough.Very powerful specialists are in Australia, New Zealand.
By the way, most of the countries of Africa, which are located below the sugar desert, have a palliative care system and children's forms of opioids. So that a palliative system also earned in our country, not only laws and orders are needed, but also interagency interaction and understanding. There are no provisions regarding the device of hospice, recommendations on the medications to the word, if there are certain achievements in the adult palliative in Kharkov, then in the children, unfortunately, we are not leaders.
In Rivne, Ivano-Frankivsk created regional programs, there are freelance specialists in palliative, and exit brigades are open on an ongoing basis. Who today does not give us at the regional level to resolve the issue with children's palliative or, for example, at least allow parents to visit children in intensive care? No one bothers. So far, this is just our reluctance.
Every day in Ukraine, a person dies from not provided medical care. In the meantime, the reform of this sphere is openly sabotaged at the highest level. A lot of innovations are declared, only nothing is done. To start a medical reform, you need to change more than 50 laws, but not one of them has been adopted yet. The situation is catastrophic. Resistance - colossal. Quite a lot of people want to work in the old framework for various reasons.
First, we must approve the reform plan and understand where we are moving. In Ukraine, so far there is only a doctrine, but my colleagues and I do not lose hope.
